Özet

Katarakt cerrahisi sonrası antimikrobiyal tedavide topikal tedavilere alternatif olarak, kontrollü salınımlı göz içi lens (GİL) implantlarının kullanıldığı destekleyici platformların geliştirilmesi giderek önem kazanmaktadır. Kontrollü ilaç salım sistemleri, ilaç dozajını ve sıklığını düşürmesi, hedeflenebilirliği sayesinde yan etkileri ortadan kaldırması ve bu özellikleriyle hasta güvenliği, tedavi ve maliyet üzerinde olumlu etkiye sahip olması nedeniyle talep görmektedir. Geleneksel tedavi yöntemleriyle mümkün olmayan ilacın daha uzun süre terapötik seviyede tutulması ve uzun süre uygulanması gibi avantajları nedeniyle son yıllarda üzerinde sıkça çalışılmaktadır. İlaçlar lipozomal sistemler kullanılarak istenen bölgeye iletilebilir, ancak stabiliteleri ilaç aktivitesini sürdürmek için yeterli olmayabilir. Bu durumda lipozomları hidrojel yapılarla birleştiren sistemler kullanılabilir. Aljinat (ALG), yüksek biyouyumluluk, düşük toksisite, mukoadheziflik ve düşük maliyet gibi özellikleri nedeniyle birçok biyomedikal uygulamada kapsamlı bir şekilde araştırılmıştır. Kontrollü ilaç salım sistemlerinin performansı hidrojel yapı üretim süreçlerinden doğrudan etkilenmektedir. Hidrojellerin yapısal özelliklerinin kolaylıkla kontrol edilebildiği üretim yöntemlerinin tercih edilmesi bu sistemlerin tasarımında önemlidir. Hem hidrojel üretiminin basitliği hem de hidrojelin yapısal özelliklerini etkileyen parametrelerin değiştirilebilmesinin kolaylığı mikroakışkan teknolojilerini tercih edilen seçenek haline getirmektedir. Mikroakışkan sistemler, düşük hacimli sıvıların akışkan özelliklerinden yararlanarak ve bu sıvıların kılcal hareketlerini kontrol ederek istenen karakterde ve kaplama formunda hidrojel üretimine olanak sağlamaktadır. Özellikle t-şekilli mikroakışkan bağlantı (TMJ) sistemleri, mikron boyutundaki kanallardan taşınan sıvıların bir araya gelmesini sağlayarak homojen dağılımlı ve istenen boyutlarda mikro kabarcıkların oluşturulmasını mümkün kılmaktadır. Bu özellikler, kaplama formu üretiminin yanı sıra tek tip gözenek ve boyut dağılımına ve jel boyunca homojen olarak dağılmış ilaç yüklü lipozomlara sahip hidrojel üretimi için uygun hale getirir. Bu tez çalışmasında, GİL implantı üzerinde TMJ yöntemi kullanılarak homojen dağılımlı moksifloksasin-içeren lipozomlar içeren ALG hidrojel kaplama oluşturulması ve MXF'nin (%1-5 w/w) kontrollü salınımı ile katarakt cerrahisi sonrası antimikrobiyal tedavi geliştirilmesi amaçlanmıştır. Üretim ve karakterizasyon aşamalarından sonra antimikrobiyal tedavide kullanılmak üzere en uygun ilaç salımı ve antimikrobiyal aktiviteye sahip hidrojel formu ve hidrojel kaplı GİL implantı seçilmiştir. Daha sonra, MXF ilacının salım davranışını incelemek için, yalnızca MXF ile kapsüllenmiş lipozomlar, MXF ile kapsüllenmiş lipozomlarla zenginleştirilmiş alginat nanokompozit hidrojel ve MXF ile kapsüllenmiş lipozomlarla zenginleştirilmiş alginat nanokompozit hidrojel kaplamalar (IOL üzerinde) üzerinde in vitro salım çalışmaları gerçekleştirilmiştir.

As an alternative to topical treatments of antimicrobial treatment after cataract surgery, the development of supportive platforms using controlled-release intraocular lens (IOL) implants is becoming increasingly important. Controlled drug release systems are in demand because they lower the drug dosage and frequency, minimise side effects due to their targetability and have a favourable impact on patient safety, treatment, and cost because of these features. It has been extensively investigated in recent years owing to their advantages such as keeping the drug at a therapeutic level for a longer period of time and applying it for a long time which is not possible with traditional treatment methods. Drugs may be delivered to the desired region using liposomal systems, however, their stability might not be enough to maintain drug activity. In this case, systems that combine liposomes into hydrogel structures can be utilized. Alginate (ALG) has been thoroughly researched in many biomedical applications due to its properties including high biocompatibility, low toxicity, mucoadhesiveness and low cost. The performance of controlled drug release systems is directly impacted by hydrogel production processes. Preferring production methods in which the structural properties of hydrogels can be easily controlled is important in the design of these systems. Both the simplicity of hydrogel manufacturing and the ease with which parameters affecting the structural characteristics of the hydrogel can be altered make microfluidic technologies the preferred option. Microfluidic systems allow the production of hydrogel with the desired character and coating form by utilizing the fluid characteristics of low-volume liquids and by controlling the capillary movements of these liquids. Particularly, T-shaped microfluidic junction (TMJ) systems enable the confluence of liquids transported through micron-sized channels, making it possible to create microbubbles with uniform distribution and in desired sizes. These characteristics make it suitable for coating form production as well as hydrogel manufacturing with uniform pore and size distribution and drug-loaded liposomes homogeneously distributed throughout the gel. In this thesis study, it is aimed to form an ALG hydrogel coating containing homogeneously distributed moxifloxacin-incorporated liposomes using the TMJ method on the IOL implant and to develop antimicrobial treatment after cataract surgery by controlled release of the MXF(1-5% w/w). The hydrogel form and hydrogel-coated IOL implant with the optimal drug release and antimicrobial activity selected for use in antimicrobial treatment after the production and characterization stages. Then, in order to examine MXF drug release behaviour, in vitro release studies performed on solely MXF-encapsulated liposomes, MXF-encapsulated liposomes enriched alginate nanocomposite hydrogels and MXF-encapsulated liposomes enriched alginate nanocomposite hydrogel coatings (on IOL).
