Gebze'de sağlık ocaklarında maliyet-etkinlik perpormans değerleme üzerine bir deneme

Abstract

Ülkemizde sağlığa ayrılan kaynakların kısıtlı olmasına karşılık sağlık harcamalarının hızlı artması nedeniyle sağlık hizmetlerinin iyileştirilmesinin en önemli unsurlarının birisi, bu kaynakların etkili kullanımıdır. Sağlık hizmetlerinin ekonomik kalkınmaya etkisi ve Temel Sağlık Hizmetlerinin toplumun sağlık seviyesine yaptığı katkı gözönüne alındığında, bu hizmetlerin temel örgüt birim olan sağlık ocaklarında kaynak israfının engel lenmesi ve sunulan hizmetin etkililiğini artırmak amacıyla kullanılan kaynakların hangi ölçüde verimli ve etkili olduğunun saptanması, sunulan hizmetlerin birim maliyetlerinin hesaplanması, sağlık ocakları maliyet yapılarının ve per sonel etkililiği ile performansının belirlenmesinin önemi ortadadır. Bu nedenle çalışmalarımızda organizasyon yapıları oturmuş, benzer sosyo- ekonomi ve demografik özellikler gösteren Gebze'de hizmet vermekte olan 5 adet sağlık ocağı seçilerek maliyet, etkililk ve performans bakımından karşılaştırmalı bir çalışma yapılması hedeflenmiştir. Çalışmanın bölümleri aşağıdaki şekilde özetlemek mümkündür. Birinci bölümde sağlık ocakları ve sunulan hizmetler hakkında bilgi verilmiştir. İkinci bölümde Maliyet, Etkililik ve Performans analizinin tanımı ve çeşitleri hakkında bilgi verilmiştir. Üçüncü bölümde ise Maliyet, Etkililik ve Performans analizinin göstergelerinin neler olduğu anlatılmıştır. Dördüncü bölümde ise uygulamanın amaç, kapsamı, metedolojisi hakkında bilgi verilmiş Sağlık Ocaklarında hizmet oranlarının belirlenmesi ve Sağlık Ocaklarında Maliyet, Etkililik ve Performans göstergeleri üzerine yapılan uygulama verileri ortaya konarak, değerlendirilmesi yapılmıştır.Since resources devoted to health sector is limited in < Turkey and also the costs of health services are increasing rapidly, the basic factor for the improvement of health services can be achieved with the efficiend use of these resources. The impact of health services to the economic develeopment and the contribution of primary health care to the society's health status is considerably important. Health units are the basic organizations of primary health care. İn order to improve the efficieney of services provided by health units and to prevent wasting of resources, the productivity and efficiency of resources, the unit costs of services provided, the cost structure and also the performance of personnel needed to be determined. For this purpose five health units were chosen to examine their costs, efficiency and performance indicators comperatively. The reason for choosing these health, units serving to the population showing similar socio-ekonomic and demoraphic charactersticks have organized structure in providing primary health care. It is possible to summarize the parts in this work as below. In the first part are given informations about the health homes and its services. In second part are explained the definitions and the types of the cost, the activity and the performance analysis. In the third part, are explained the statements of the cost, the activity and the performance analysis. In the fourth part are given informations about the target, the covarage and the method of the practical example which is chosen. And a general exami nation is made by determinig the service rates in the health homes and by putting forward the practical data of the statements about the cost, the activity and the performance analysis.
